Re-registration forms checklist

Please ensure all forms are returned with your re-registration fees. Unfortunately, we are unable to accept incomplete registrations

1. Complete and return the following forms:

    • Re-registration Form. (one per family)

    • Student Emergency Identification Form. (one per student)
      If the contact names for alternates are the same as the previous year you do not need to get signatures.
      Please note these Student Emergency Identification Forms are only required once a year at the time of re-registration and will not be requested at the beginning of the school year. Therefore we suggest you keep a copy at home and if there are any changes we would request you to notify the school office.

    • Student Release Form.(one per student)
      If the contact names for alternates are the same as the previous year you do not need to get signatures.
      The same procedure will apply as per the above Student Emergency Identification Form.

2. Attach a cheque.

      • Local Re-Registrations: $150.00 per student or $200.00 per family.
        International Re-Registrations: $250 per student or $300 per family.
